Abstract:
The present cumulative dissertation, titled "Imaging and clinical risk stratification of poststroke epilepsy," explores key clinical and imaging biomarkers to enhance the understanding of poststroke epilepsy (PSE) risk in patients treated with mechanical thrombectomy (MT) for large vessel occlusion stroke. The first study, "Frequency and predictors of poststroke epilepsy after mechanical thrombectomy for large vessel occlusion stroke: Results from a multicenter cohort study," investigates clinical and imaging predictors of PSE. Conducted across two tertiary care stroke centers, this study included 348 patients treated with MT, followed for a median of 6.5 years. Findings revealed that PSE developed in 9% of the cohort. Larger infarct size and the presence of cerebral microbleeds were identified as independent predictors of PSE, emphasizing the importance of comprehensive neuroimaging analysis for risk prediction. The second study, "Voxel-based lesion symptom mapping to predict poststroke epilepsy after mechanical thrombectomy," focuses on the topographical mapping of lesion locations associated with PSE. Utilizing advanced MRI techniques, this study analyzed the lesion maps of 251 patients, identifying specific brain regions linked to PSE risk, such as the gyrus rectus, orbitofrontal gyrus, and temporal opercula. These findings contribute to a better understanding of the anatomical underpinnings of PSE and provide insights for personalized follow-up care. Together, these studies advance the field by highlighting critical imaging and clinical markers that refine risk stratification, enabling improved prediction and management of PSE in stroke patients undergoing MT.
