Quality of Life after TAPP Hernia Repair: Long Term Results

Abstract:
Introduction: In hernia surgery laparoscopic operation methods, such as transabdominal preperitoneal patch plastic (TAPP), gain more and more popularity in recent years. However, long term results for TAPP hernia repair, especially data on health related quality of life (HRQOL) are lacking. The aim of this study was, first, to compare long-term postoperative HRQOL data of patients undergoing TAPP repair for inguinal hernia (IH) with data of patients undergoing TAPP repair for the sportsmen’s groin (SG), and second, to compare these data with data of an sex- and age-matched normal population. Material and Methods: We included all Patients (n = 559) who underwent TAPP hernia repair between 2000 and 2005 at the district hospital of Weiz (Austria, Styria). Five-hundret-twelve patients (91.6%) were treated because of IHs and 47 patients (8.4%) were treated because of SGs. We sent out a self-designed hernia questionnaire, to evaluate postoperative pain and recurrence rate and the short-form 36 health survey (SF-36) for HRQOL assessment. Results: Finally, 383 completed hernia questionnaires (69% response rate) were available – 355 from the IH- and 28 from the SG-group. Patients’ mean age was 62 ± 16 years. The mean follow up (FU) time was 94 ± 20 months. There were no statistically significant differences in HRQOL between the IH FU group and the sex- and age-matched normal population. There were statistically significant differences in HRQOL between the SG FU group and the IH FU group in 8 domains of the SF-36, and between the SG FU group and the sex- and age-matched normal population in 3 domains of the SF-36, showing better results for the SG FU group. The total recurrence rate was 6.5%. The postoperative pain at the FU was 0.9 ± 1.7 at the VAS. Discussion: Long term results of HRQOL after TAPP repair for SG and IH are comparable with those of the normal population, even with better results for SG patients.
