- Background. For the experienced emergency physician as well as for the beginner, pre-clinical treatment of altered mental staus can be very challenging because of the heterogeneity of its causes. Results. Contrary to the diagnostic possibilities of an emergency department, many diagnostic tools are not available under pre-clinical conditions. After initial assessment of immediate threats by using the ABCDE approach, it is important to identify those pathologies where the patient would profit from further immediate and specific pre-clinical treatment. Situations where a specific hospital has to be chosen (24-h CT scan availability, stroke unit, neurosurgery department) must also be recognized. Objective. The authors give a compact and structured summary about relevant key points in the treatment of nontrauma-associated altered mental status. Basic and advanced life support is assumed in each case and not covered by this article.
